Changes in Sexual Function after SCI
Erection - Reflexogenic erection is when touching, rubbing or deep pressure causes an erection through a feedback loop in the nervous system between the genetalia and the sacral spine.  Psychogenic erections are those influenced by mental stimuli.  Reflexogenic erections are possible and even, enhanced in men with quadriplegia because the injury is far above this feedback loop but, psychogenic erections are not.  Men with injury below T10 but above S4-5 will not have reflex or psychogenic erections.  Men with injury below S4-5 will still have psychogenic and possibly partial reflxogenic erections. 

Ejaculation and Orgasm - Men wtih SCI above T10 should still have the capacity for ejaculation but may require special sperm retrieval methods to do so.  If the injury is incomplete, they may also experience orgasm but it may be weaker or different than prior to the SCI.  Some men with complete injuries have reported the ability to perceive an orgasm.  Men with SCI below T10 generally do not have the ability to ejaculate.  Again, orgasm depends of the severity of the SCI. 

Despite the level or completeness of SCI, sexual needs and desires are still present.  People with SCI have the same needs for love, affection and sexual contact as the uninjured.  People with SCI can have satisfying sexual relationships and can also be biological parents.  The role of the caregiver may include assisting with set-up of a sexual environment.  This set-up might include administering a medication, assisting with pericare, clothing removal or adjustments, positioning and placement of assistive devices within reach.  It is important to approach this role with professionalism and empathy.
